C ANCER The scope of the problem in the United States is large. Some 1.2 million people were diagnosed with potentially fatal cancer in the year 2000. Of these, 59 percent were expected to live for at least five years (in some, the cancer may be continuously present for more than five years) with or without evidence of cancer. People of all ages, from birth to advanced age, can manifest cancer, making it the second-leading cause of death in the United States. In children cancer is unusual, but it has consistently been the leading cause of death from disease. As mortality rates from cardiovascular disease decline, the proportion of cancer deaths increases. It is anticipated that the mortality rate from cancer will surpass that from heart disease by the year 2050. Direct and indirect financial costs of cancer in the United States for the year 2000 were $178 billion.Developing countries represented 80 percent of the worlds approximately 6 billion people in the year 2000. In these countries, cancer has grown from a minor public health issue in the early 1990s to a rapidly expanding problem by the beginning of the twenty-first century. The emergence of a middle class, with attendant changes in lifestyle, increased longevity and exposure to potential carcinogens, and expectations of improved medical delivery systems have fueled the growing impact of cancer in the third world. The financial resources and socio-medical infrastructure needed to diagnose and treat, much less screen and prevent these cancers, are lacking in the developing world.

The authors note that there have been 13 studies which used prospective randomized control designs which compared hypnosis either to baseline level noiception or to a control condition of some sort. These studies consistently demonstrate that hypnosis can assist patients with chronic pain with a reduction in their pain experience and pain related coping for a variety of chronic pain conditions. Hypnosis was generally found to be more effective than other nonpharmacological interventions such as physical therapy, attention, and psychoeducative strategies. The intervention which was most commonly employed was training in self-hypnosis. The authors provide a critique of these studies in that there appears to be a lack of standardization in the uses of hypnotic interventions in these clinical trials and also that the treatment group numbers tended to be lower than one might desire. The authors also provide a number of very useful suggestions for future research and clinical application of hypnosis to assisting patients with chronic pain syndromes. Morgan D. (1996). The principles of hypnotherapy. Bradford UK: Eilidon Press. The book is divided into three parts. Part A consisting of 9 chapters covers foundational material on systems theory and how that can be applied to organic systems and to hypnosis. It discusses positive feedback loops that amplify the process and negative feedback loops that subdue the process. Positive feedback amplification can be either beneficial or harmful depending on whether increasing the effect is desired, while negative feedback calming can be either beneficial or harmful depending on whether decreasing the effect is desired. Morgan focuses on the activity of a system, the activity of its component subsystems, or the activity of the supersystem of which the system is a component. His main theoretical focus is on whether the activity of the system is increasing, decreasing, or staying the same, as other components change in specified ways. Hypnosis is described in systems terms, not as a “state,” but rather in terms of what system or subsystem components are changed, either increased or decreased, during trance induction, deepening, maintenance, trance work, and termination. He uses simple symbols to represent activities in quasi-mathematical style, which is helpful. Some of the letter symbols are used for different activities in different parts of the book. It would have been easier to follow if given letter symbols had been used more consistently to represent single specific activities. Some of his textual references to others publications are found in the bibliography at the end of the book, but some are unfortunately not present in the bibliography. He goes through common hypnotic tests in Chapter 8 and inductions in Chapter 9 explaining them in systems terms. This does produce some greater conceptual clarity with respect to subsystem component activities during hypnosis. Part B consisting of 6 chapters is the central discussion on what the systems approach can teach us about hypnotherapy. Chapter 10 discusses the matter of diagnosis in systems terms looking at precursors, problems, and resultants, and how they change with respect to one another. Chapter 11 focuses on positive and negative feedback loops in organic systems including behavioral, emotive, and cognitive. The vicious circle is a positive feedback loop with harmful consequences. Thinking in systems terms may make needed changes clearer to the therapist. Chapter 12 looks at the consequences of symptom reduction. The remaining three chapters in this part examine making changes by choosing the appropriate positive feedback loops to amplify desired effects and the appropriate negative feedback loops to quiet undesired effects, planning the changes, and reinforcing the changes by selecting natural reinforces.

As a family man and an electrical engineer, Rick Taylor wouldnt consider himself a trendy type of guy. And yet when he spent $7,500 recently for a do-it-yourself kitchen remodel in his Corona, Calif., tract house, he followed some strong trends in the home improvement industry. For instance, theres major remodeling going on in the Seattle area, notes Appraisal Institute associate appraiser Nanette Lloyd. It seems to be more the trend than new construction. Part of the reason for the remodeling boom, she explains, is the limited amount of space in the city for new homes.

Union Blvd. The department is offering free tetanus shots to adults next week, April 3-7. A tetanus booster is recommended every five to 10 years for adults.
Monday May 5, 2003 will go down in history as a day to remember in employment law. It was the day the long-debated amendments to the Health Safety in Employment Act finally came into effect. Why is this new legislation so important? The term “workplace stress” took on a whole new significance, with new obligations for employers, and potentially serious financial consequences for those who ignore them. Workplace stress is no longer simply a sad fact of life. It is now an officially recognised and legislated “workplace hazard”, with the amendments extending the definitions of “harm” and “hazard” to include physical and mental harm caused by work-related stress. A cursory reading of the new legislation might be enough to instantly treble employers workplace stress, but a deeper understanding goes some way to restoring mental equilibrium. The Occupational Safety and Health booklet, Healthy work, Managing Stress in the Workplace, tells employers that “there is only a requirement to manage work stressors or the individuals stressed situation where you can be reasonably expected to know about the stress. Even then the obligation is only to do what is reasonably practicable in the circumstances.”

With gusts of over 30 knots blasting over the Holy Loch, several dinghy sailors found it nigh on impossible to get their boats righted in the difficult conditions. In the slow dinghy class, Strathclyde Loch Sailing Clubs Crawford Reid produced two firsts in his Albacore, putting him out in front as a clear leader in the 13-strong class and Alan Milton in Pepsi took two more decisive wins in the Sigma 33 class. Cove Sailing Clubs Alan Harper in Pied Piper sealed a championship win with a second and fourth, while Clyde Cruising Clubs Donald Sharp had two good wins and lifted the IRC Class 2 crown.
Squalls ripped through the southern Pacific yesterday, knocking down at least three of the 13 yachts still competing in the BOC round-the-world solo race. An American, Arnet Taylor Jnr, and two Britons, Robin Davie and Harry Mitchell, reported being knocked over. Christophe Auguin, of France, turned in another 300-mile day to remain solidly in the lead. His countryman Jean Luc Van den Heede, however, sailed a straighter course to cut Auguins advantage down to 142 miles from 239 just two days earlier.
Morning Glory, a German yacht, was the first of 100 boats to complete the 1996 Cape Town to Rio sailboat race. The 3,429 nautical miles (6,350km) voyage took 14 days 14 hours 52 minutes.

WITH ninety per cent of dieters regaining all or part of their lost weight within five years, there is huge pressure to find new health and beauty regimes that last. And no matter how many times we tell ourselves that no one is perfect we still cant help hoping that one day we will be happy with our bodies. We model our dreams for our bodies on celebrities and people in the public eye but rarely do we hear about their imperfections such as bunions, wonky toes, cellulite or wrinkles.

In Japan, textile products in a new category helpful to beauty and health have made their appearance. Fuji Spinning Co., Ltd. (Fujibo) has commercialized its vitamin-finished fabric “V-UP” and that was the start of this phenomenon. Subsequently, each firm has presented textile products with aromas to create a good feeling or with special ingredients to make the burning of fat easier. Later, this boom disappeared. Then in 2001, Fujibo announced “V-UP”. Its catch phrases in selling were “wearing vitamins” and “Vitamin C-formulated fabrics.” Vitamin C keeps the generation of melamine in the human body under control and exhibits skin whitening effects. Vitamin C functions to promote the synthesis of collagen and to restrain the decomposition of collagen, while it is effective in preventing wrinkles on the skin. The anti-oxidation effects exhibited by the sebum can also ward off sunburn. Even so, Vitamin C itself is unstable because it is weak against heat and oxidizes against light. Consequently, a pro-vitamin which precedes Vitamin C is used in manufacturing. Being oil -soluble, a pro-vitamin fixed onto fabrics is absorbed into the skin. The following mechanism is the change of the pro-vitamin into Vitamin C by various internal enzymes after its entry into the human body.

Golfs governing bodies are to introduce a one-stroke penalty rule in an attempt to combat slow play. The words “slow play” are to appear in the rulebook for the first time from 1 January. Among the other changes coming into effect next year are ones that permit committees to make local rules to prohibit play from an environmentally sensitive area. A further amendment will deny a player relief when his stance, not the ball, is affected by casual water, ground under repair or certain damage to the course.

SAVING money on your credit-card bill can be as simple as switching to a new card company. It is not just a question of playing the field for the lowest interest rate or the lowest annual fee. Some card issuers will trim your overall level of debt if you transfer your credit card business to them, even if you are still in the interest-free period and not yet paying any interest on that debt. Midland Bank also offers a 5 per cent rebate on a balance transferred from a rival credit-card company. But this rebate is only available to people who are already paying interest on at least some of the transferred debt. If you are simply transferring a balance that has not yet attracted interest, there is no rebate. The maximum saving in moving to a Midland Access or Visa card is pounds 100. The Bank of Scotland runs a similar scheme for new customers who consolidate their credit and storecard debt on to one of its cards. A sliding scale offers a maximum saving of pounds 60, for debt transfers of pounds l,250 or more.
WASHINGTON (AP) — Many consumers have started to pay off their credit card debts more quickly and to resist taking new cards, helping reduce record-high personal bankruptcy rates, a consumer group said Tuesday. The Consumer Federation of America said 112,000 fewer people filed for and received bankruptcy court protection last year than in 1998, which it termed the largest one-year decline on record. “Consumers have increasingly just said `no to credit card solicitations,” Stephen Brobeck, executive director of Consumer Federation, said at a news conference. In addition, he said, many consumers with credit card debts have realized the importance of paying more than the minimum payment on their monthly bills and are sending heftier payments.
Overall about 17 percent of Americans are either very or moderately worried about not being able to make the minimum payments on their credit cards. Another 22 percent say they are not too worried, and 46 percent say they are not worried at all.

Benchmark initiatives can lead to reduced costs and increased productivity throughout the customer services arena. procedures that directly and indirectly influence wverall performance. To ensure that benchmarking among companies is meaningful anc accurate, these differences must be taken into account up front. For example, in the area of credit and collections, companies must consider the effects of climate on their performance. Utilities that operate in northern climates typically have winter-disconnect moratoriums, while those in the South do not. Not being able to disconnect a customer for nonpayment over a four-to-five-month period can wreak havov on performance indicators. In such an example, only utilities with winter-disconnect moratoriums ought to be compared with each other. (See Sidebar, “Defining Metrics.”) Likewise, billing can present various uniformity challenges. Companies that use multivarious billing systems must be separated according to what systems are most similar in order to produce an apples-to-apples comparison. The result of such an analysis may well be that a meaningful comparison is simply not possible because of multiple and complex differences in operational management. But for most utilities, their customer service operations share enough similarities upon which to perform useful-and potentially lucrative-benchmarking.
MeasurementBritish mathematician and physicist William Thomson (1824#x2013;1907), otherwise known as Lord Kelvin, indicated the importance of measurement to science:Possibly the most striking application of Kelvins words is to the explanation of combustion by the French chemist Antoine Lavoisier (1743#x2013;1794). Combustion was confusing to scientists of the time because some materials, such as wood, seemed to decrease in mass on burning: Ashes weigh less than wood. In contrast, others, including iron, increased in mass: Rust weighs more than iron. Lavoisier was able to explain that combustion results when oxygen in the air unites with the material being burned, after careful measurement of the masses of the reactantsair and the material to be burnedand those of the products. Because Lavoisier was careful to capture all products of combustion, it was clear that the reason wood seemed to lose mass on burning was because one of its combustion products is a gas, carbon dioxide, which had been allowed to escape.

WASHINGTON — Sales of existing homes fell for the sixth straight month in January, dropping to the slowest sales pace on record. Median home prices were also down, and many analysts predicted further price declines in the months ahead, given high levels of unsold homes. The median price of a home sold in January slid to $201,100, a drop of 4.6 percent from a year ago. Particularly alarming, analysts said, was the fact that the inventory of unsold homes jumped to a 10.3 months supply, meaning it would take that long to sell the 4.19 million homes on the market at the January sales pace.
WASHINGTON (Bloomberg) — U.S. sales of new homes rose in October to the highest level in three months as low mortgage rates and stock market gains rejuvenated housing activity. “The market is going to remain as strong as its been, which is an incredibly healthy home buying market,” said Lori Steiner, senior vice president of marketing at Dominion Homes in Dublin, Ohio. A rebound of new home sales in the West accounted for the October increase, as sales fell in the other three regions. Including sales of previously owned homes, “the housing market is going to set a third consecutive record this year,” said David Berson, chief economist at Fannie Mae, the worlds leading buyer of single-family home mortgages. Home sales next year “will be extremely strong.”

Building on the premise that empowerment of others is a viable expectation for effective administrators, Thomas Harvey of the University of LaVerne and Bonita Drolet of the Placentia Yorba Linda, Calif., Unified School District have written a book on implementation strategies in this critical area. Building Teams, Building People focuses on the concept that true success in any venture, particularly education, is measured in terms of effectively uniting groups of empowered people in a common purpose. The book is readable, crisply written, and well documented. It provides tables, charts, and usable instruments with prescriptions for the team-building process. The authors often reference personal experiences in their roles as organizational leaders.

Management, consultants, and trainers think you can generate strong team feelings with a simulated project or a social event. Its worth a try, but far from easy to come up with a team-building event that doesnt cause more trouble than it solves. For example, picture Dilbert, Tina, Alice, and the gang accidentally feeding a teammate to a bear.
Building unbeatable teams is the key to successful organizational growth. The Academy of Medical-Surgical Nurses (AMSN) has been successful for the last 12 years because we have learned the benefits of team building. Although our board is made up of eight volunteers who are elected by our members, we know the value of accomplishing our vision and strategic plan can only be operationalized by being team players. AMSN is like a flock of geese. If you ever see birds flying south for the winter, you may notice that they fly in tandem, with any one in the flock being capable of taking the lead in various situations. To build a team, partnerships must be developed among team members based on integrity and mutual respect, work groups must be developed that stimulate people to reach their highest potential, and pride must be taken in serving others. AMSN has determined that the objective of our team is to serve our constituents by being ever mindful of our mission, vision, and strategic plan, which is a work in progress. Monday May 5, 2003 will go down in history as a day to remember in employment law. It was the day the long-debated amendments to the Health Safety in Employment Act finally came into effect. Why is this new legislation so important? The term “workplace stress” took on a whole new significance, with new obligations for employers, and potentially serious financial consequences for those who ignore them. Workplace stress is no longer simply a sad fact of life. It is now an officially recognised and legislated “workplace hazard”, with the amendments extending the definitions of “harm” and “hazard” to include physical and mental harm caused by work-related stress. A cursory reading of the new legislation might be enough to instantly treble employers workplace stress, but a deeper understanding goes some way to restoring mental equilibrium. The Occupational Safety and Health booklet, Healthy work, Managing Stress in the Workplace, tells employers that “there is only a requirement to manage work stressors or the individuals stressed situation where you can be reasonably expected to know about the stress. Even then the obligation is only to do what is reasonably practicable in the circumstances.”

AUTUMN-WINTER 2005 is a watershed season for the house of Versace after the annus horribilis of 2004 for the creative director, Donatella Versace. It is seven years since the founder, Gianni Versace, was murdered outside his mansion on Miami Beach and though his little sister has scored personal triumphs with the womenswear collections, Versace for Men seemed to be floundering. Even “friend of the house” Sir Elton John defected to Yohji Yamamoto.
MY FRIEND Ian Sturrock once went to a fancy-dress party as the Menai Straits. He wore a sack, to which he attached rusty tins, seaweed, shells, bits of driftwood, used condoms and dead crabs. To complete the tableau, he put a kipper under each arm. He didnt win a prize or even get many dances. But he said that everyone looked at him. He was well pleased. But even Ian would have been staggered at the total looniness of the Australian Neil Wilson, the man who wanted to be a fish. Wilson has just been found dead at Toolondo, near Victoria. Strangely, he did not drown, but suffocated in the fish-shaped bodysuit he had made from heavy green plastic sheeting. Unfortunately, he had clearly not read as far as chapter three in How To Be A Fish, because in the costume he had forgotten to put in ventilation slits. He was discovered by a passer-by in a field close to Wilsons favourite reservoir, “where he was a familiar sight at the waters edge, swinging from a rope while pretending to be a fish”, my cutting says. Im afraid I cant tell you any more. Thats a shame, because this intriguing tale clearly raises more questions than it answers.

SAVING money on your credit-card bill can be as simple as switching to a new card company. It is not just a question of playing the field for the lowest interest rate or the lowest annual fee. Some card issuers will trim your overall level of debt if you transfer your credit card business to them, even if you are still in the interest-free period and not yet paying any interest on that debt. Midland Bank also offers a 5 per cent rebate on a balance transferred from a rival credit-card company. But this rebate is only available to people who are already paying interest on at least some of the transferred debt. If you are simply transferring a balance that has not yet attracted interest, there is no rebate. The maximum saving in moving to a Midland Access or Visa card is pounds 100. The Bank of Scotland runs a similar scheme for new customers who consolidate their credit and storecard debt on to one of its cards. A sliding scale offers a maximum saving of pounds 60, for debt transfers of pounds l,250 or more.

While on a fox hunt, Cig is led back in time to her ancestral home in a fantasy sequence, and learns more about what is truly valuable to her.
One of the most interesting publications in shotgun literature is entitled The Mysteries of Shotgun Patterns, written by George Oberfell and Charles Thompson. Published in 1957 through Oklahoma State Universitys Division of Engineering Research, this unpretentious book is the compilation of the extensive research conducted by Oberfell and Thompson to determine why shotguns do what they do. When I was asked by Germanys DEVA laboratory–that countrys independent testing and proofing laboratory for sporting firearms–to obtain a copy of the study for them I didnt realize how rare the book had become, with asking prices exceeding $100. Considering that probably very few of our readers have had an opportunity to ever read the study, I thought I would share with you some of the gems of shotgun esoterica the authors discovered. For example, “a smooth, mirror-like finish on the inner surfaces of the choke bore generally lowers pattern efficiency.” Would you ever have believed that? Yet, the authors document numerous tests that compared highly polished choke tubes to less polished choke tubes, and statistically, the unpolished tubes delivered more efficient patterns with fewer open patches. This also proved true of tubes that were knurled inside and tubes that were cut near the muzzle end with a series of interior grooves .003″ in depth and 1/8″ wide.

PRAYER IS, ABOVE ALL, A GENUINE ENCOUNTER with the Lord and with others too. It is a true relationship. A true relationship can be scary. One is afraid to fail in a relationship because of the fear of rejection. Also, one is afraid to succeed because of the many changes needed to adapt and adjust to the new situation. In a relationship, while keeping company with the significant other, one has to accept the consequences of this company. We often think that God is a miracle worker, one who magically transforms war into peace, makes the paralyzed walk, and provides homes for the homeless. He makes all things right. We expect a miracle at the particular moment we find ourselves in difficulty. We pray as though we were eloquently presenting a case before the Almighty to persuade heaven to create a totally different course of events.
In considering the work place, I think my friend Patricia Wyatt, the President of North America for HIT Entertainment, expressed perfectly what we should all strive for when she said, “Whatever my position, Ive worked with all my heart to try to create environments where people feel valued, honored, creative, and part of something that supports them as human beings. While I may not have reached these lofty goals, I continue to do all I can to humanize the workplace. I try to make the motions of my workday filled with artful gestures-thoughtfulness towards my co-workers, and striving to achieve the best results through thoughtful means. A COUNCIL has rewarded its 6,000 staff for their hard work by giving them alla pounds 1.34 mug. Tower Hamlets Council in East London made no apology for sending the mugs, saying pay rises were not readily available in the public sector. An official said: We make no apology for spending pounds 1.34 each on our staff. They all deserve it.
Customers caught in the strike have complained that as soon as BAs rank and file walked out of Heathrow, its managers vanished. Halloween is coming. Time to put on a costume and have some fun. But some animals get to dress up almost every day–not for fun, but for survival. Its a fish-eat-crab world out there. Octopuses, starfish, and lobsters also love to dine on crabmeat. But the decorator crabs pictured here have a tricky way to cover up and hide from their enemies. They simply “decorate” themselves with parts of plants or animals from the seafloor. And then, ta-dah! They can scurry along almost unseen. Other kinds of animals, too, use “dressing up” as a way to hide from predators.
In the days leading up to Halloween before Hurricane Katrina, a security guard stood outside the door of Uptown Costume and Dancewear while lines of anxious customers stretched two blocks down Magazine Street waiting to be ushered inside. “But rules are rules,” the guard would say. “Only 20 in at a time.” Inside, the lucky few clutch bat wings and corsets and mini- dress nurse uniforms while waiting in an hour-long line to reach the register.

Affordable housing. Low cost of living. High quality of life. The combination of these three things sets the Fort Wayne housing market apart. A recent home price analysis by the National Association of REALTORS(R) revealed that the Fort Wayne housing market is in excellent shape, with a potential for significant housing equity gains, a very favorable home price-to-income ratio and an even better mortgage servicing cost-to-income ratio. The same report shows that the region added an estimated 15,000 new housing units in the past five years, approximately 12,000 of which were single-family dwellings. New developments continue to pop up in suburban areas surrounding the city. Conveniently located medical facilities, schools, libraries, parks, retail stores and restaurants add to the sense of community.
Prosperity in the U.S. construction industry was again on the house, as the residential segment continues to exhibit most of the recent growth in the domestic building industry. The construction industry appears to be hovering at a level close to last years pace, says Robert A. Murray, vice president of economic affairs for McGraw-Hill Construction. Housing remains very healthy, and the upturn for non-residential building in February was a welcome development after a sluggish January. Murray says that higher steel prices could be putting a crimp in the nonresidential segments. Over the course of 2004, its expected that nonresidential building will see a modest strengthening trend. Admittedly, though, this years sharp increase in steel prices has made the nonresidential upturn less certain, given the impact that higher costs and materials shortages may have on individual construction projects. At this juncture, it appears that the nonresidential upturn will be dampened but not derailed, assuming steel prices settle back by midyear, but the situation clearly bears watching.

Are your clients retirement plans on track? Academics have concluded that retirees generally can plan on fixed annual withdrawals of between 6 percent and 8 percent of a portfolio, assuming 50 percent to 75 percent of assets are kept in large-company stocks and the balance in high-grade corporate bonds. Thats a 4 percent to 5 percent withdrawal rate if you are adjusting annually for inflation.
Early retirement, or retirement at any age, may be less than an enjoyable experience without sufficient income. Generally, financial planners recommend that an individual should strive to maintain 70 percent to 80 percent of his or her final working years income during retirement and have made provisions to increase income in retirement to keep up with inflation. A small but growing number of financial planners recommend that individuals should strive for 100 percent or more income replacement during retirement years. (3) The logic behind this recommendation is that individuals tend to spend more money when they have time to do what they would like to do. In planning for retirement at any age, the first task is to determine the income desired in retirement and examine how those income goals will be met. The exact amount of income that an individual needs in retirement is specific to each individuals financial circumstances and goals. For example, an individual or couple with a home that is paid for would li kely spend less on housing in retirement than an individual or couple who are still paying off their home or renting. Financial planners generally suggest that an individuals retirement plan include at least three important sources of retirement income. These sources usually include Social Security benefits, personal savings/investments, and employer/employee retirement accounts, such as defined benefit plans, 401(k)s, 403(b)s, and IRAs. Of the aggregate income received by retired Americans, Social Security benefits consist of 42.5 percent; individual savings and investments generate 36 percent; and employer/employee retirement benefits make up 21.5 percent of retirement income. (4) The anticipated income from each of these sources should be carefully examined prior to making the decision to retire.
Employee retirement plans are a hot social issue. Future Social Security fund availability is uncertain and defined benefit plans, typically referred to as pensions, are less common. 401k has become a generic term for employer-sponsored retirement plans. But, depending on the type of company sponsoring the plan and specific plan design considerations, other types of plans might be more appropriate. Each type of employer sponsored plan also has its own specific variables and must comply with rules and laws established by the Internal Revenue Service (IRS) and the Department of Labor (DOL). When an employer puts together a 401k plan, “it is definitely not a do-it-yourself situation,” says Marilyn V. Manzer, CPA, QPA, QKA, and vice president of Baden Retirement Plan Services (BRPS). “The consequences for doing it wrong are severe and could include heavy civil or criminal penalties.”
